With our new Electronic Medical Record system, you can send us private, HIPAA compliant messages through UCSF MyChart. If you need to cancel or reschedule your appointment, remember to call us at least 24 hours in advance. If you fail to call 24 hours in advance, you will be billed the $50 no-show fee.
Request for old records - Print, fill out this form and then send it to us to have your records sent to another physician. There is no fee when records are sent to another physician. However, there is a fee when records are for personal use or a private third party. The fee is based on the recommendations offered by the Medical Board of California.
Due to COVID-19, please expect delays in processing and preparing your sublingual immunotherapy allergy drops. In order to get your vials when needed please submit your SLIT renewal paperwork early and expect a turnaround time of 2 weeks.
Our office administration location for Xolair has been reestablished at 180 Montgomery St., #2370, San Francisco, CA 94104, under the supervision of Dr. Davidson. Please call (415) 433-6673 to schedule your next Xolair appointment at this location.
As a courtesy, our staff will continue handling all Xolair orders and shipments until further notice. We will automatically place your next order once your supply runs out, so there is no need to call your specialty pharmacy to schedule your next delivery.
